Patient assumes responsibility for any and all charges incurred during their visit that may not be covered. “Non covered services” may include, but are not limited to: Refraction, Color Fundus Photo, and Contact Lens Evaluation & Fitting. Check with your provider for accurate/current coverage information, and if needed, please have referrals ready upon visit. Includes additional testing to provide the most accurate measurements for proper fitting as well as to assess the ocular system to ensure a healthy fit. When fitting contact lenses, our emphasis is on providing the best comfort and visual clarity, while maintaining good ocular health and patient convenience. We fit all types of contact lenses, including daily disposables, tinted lenses, toric for astigmatism, bifocals, and rigid gas permeable lenses.
